The Pros (for Cleared Indications)

A defined, recognized list of uses

Hyperbaric oxygen therapy has been used in medicine for decades. The FDA has cleared hyperbaric chambers for specific uses, the Undersea and Hyperbaric Medical Society (UHMS) publishes a list of recognized indications, and Medicare covers 15 indications when criteria are met. Cleared uses include selected diabetic foot ulcers, delayed radiation injury, compromised skin grafts and flaps, refractory osteomyelitis, sudden sensorineural hearing loss, severe burns and several emergencies. It adds to standard care

HBOT is used as an adjunct. For a diabetic foot ulcer, for example, it may support healing alongside wound cleaning, offloading, infection control and blood-sugar management. It works with your existing doctors, not around them.

Non-invasive and generally well tolerated

There are no incisions or needles involved in a session. You rest in the chamber and breathe. In one monoplace series, the overall rate of adverse events was about 0.4%, and most side effects are mild. Read about side effects with incidence figures.

Measurable goals

Progress is tracked with objective measures such as wound size or hearing tests, and plans are reviewed at intervals. Medicare, for instance, requires diabetic wounds to be re-evaluated every 30 days during HBOT. That makes it easier to know whether to continue.

Research support for several uses

In a 2010 double-blind randomized trial of 94 people with chronic diabetic foot ulcers (Löndahl and colleagues), 52% of the HBOT group had complete healing at one year, compared with 29% on placebo. The RICH-ART trial (2019) found greater improvement in urinary symptoms for radiation cystitis with HBOT than with standard care. For sudden hearing loss, a 2019 clinical guideline lists HBOT with steroids as an option early after onset. These are study findings, not promises, and the limits are covered below.

The Cons

A big time commitment

A typical course is 20 to 40 sessions, usually once a day, five days a week. Each session takes about 90 to 120 minutes including pressurization and depressurization, plus travel and check-in. For people who work or care for others, that can be the hardest part. Read how many sessions people typically need.

Cost if it isn't covered

At the national range of $150 to $450 per session, a 20-session self-pay course comes to roughly $3,000 to $9,000 and 40 sessions to roughly $6,000 to $18,000. Those are ranges, not quotes. Uses outside the covered list are self-pay everywhere. See what it costs.

Side effects

Middle-ear barotrauma, a pressure injury to the ear, is the most common complication. Temporary nearsightedness can develop over a course and usually improves within weeks to months after it ends. Fatigue and sinus pressure can occur. Confinement anxiety accounts for about a quarter of reported adverse events. Serious effects are rare: an oxygen-toxicity seizure occurs in roughly 1 in 5,000 to 10,000 treatments, and lung barotrauma is rare.

Contraindications

An untreated pneumothorax (collapsed lung) is the one absolute contraindication. Others need a physician's review, including certain chemotherapy drugs, COPD with air trapping, seizure disorders, recent ear or sinus surgery, colds, fever, pregnancy, and implanted devices. Not a treatment for everything

The FDA says HBOT is not cleared for conditions including cancer, autism, Alzheimer's disease, stroke, brain injury, multiple sclerosis, Parkinson's disease, depression, migraine, heart disease, sports injuries and COVID-19. Even for cleared uses, some benefits are uncertain: a 2015 Cochrane review found HBOT improved diabetic ulcer healing at six weeks, but not clearly at one year, and its effect on major amputation was uncertain. It needs medical-grade equipment and oversight

The benefits studied for cleared uses come from near-100% oxygen at 2.0 ATA or more. Soft "mild" chambers at about 1.3 ATA on air don't deliver that dose, so the pros above don't transfer to them. Read about soft vs. medical-grade chambers.

Claustrophobia and ear clearing

Myths on Both Sides

HBOT attracts both hype and suspicion. Here is where each side gets it wrong.

"It fixes almost anything" vs. "It's a scam"

Neither is true. HBOT has a defined list of cleared uses supported by research of varying quality. Marketing it for everything overstates the evidence; dismissing it entirely ignores the conditions where it is recognized by the FDA, UHMS and Medicare.

"It's FDA-approved" vs. "It's not approved at all"

Hyperbaric chambers are FDA-cleared medical devices for specific uses. Calling HBOT "FDA-approved" in general is inaccurate, and so is saying the FDA doesn't recognize it for anything.

"More oxygen is always better"

Oxygen is a dose-dependent treatment. Too much, for too long, can cause oxygen toxicity, including rare seizures. That's why pressure and session length are set by a physician and time-limited.

"A soft chamber at home is the same thing"

"You're sealed in with no way out"

In a medical center, trained staff are in communication with you throughout the session and can respond if you feel unwell or anxious. Tell them right away if you're uncomfortable.

"It's dangerous"

It isn't risk-less, but with screening and physician oversight, serious complications are uncommon. Most side effects are mild, such as ear pressure, and the rare serious ones are the reason screening exists.
